Obtaining a Medical Record

The Health Information Management department (HIM) maintains medical records as a suppot service of Northern Hospital of Surry County.

Copies of medical records may be picked up in HIM. You may call 719-7006 to make an appointment to pick up medical records. Please allow 72 hours advance notice and bring a picture ID.

Charges for copies are as follows for areas other than for continuation of care.   

Pages 1-25 are 75 cents per pages
Pages 26-50 are 50 cents per page
Pages 51+ are 25 cents per page.
